Since its start in 2018, BPCI Advanced has made significant strides. The model has faced challenges, adapted to regulatory changes, and made thoughtful model adjustments to better align value with care. As we close out Model Year 7 and prepare for the last year of the model (Model Year 8), it feels as though this year represents the culmination of these efforts. Here is a reflection of our collective efforts in 2024:

Advancing Value-Based Care - Participants in BPCI Advanced have implemented innovative strategies that transformed care delivery across a wide range of settings. These efforts have not only streamlined processes but have also fostered stronger collaboration across care teams and improved patient outcomes.

Catalysts for Change - By engaging in the model, organizations have embedded a culture of continuous improvement, leveraging data and analytics to drive decision-making and accountability.

Building Resilience - The model has supported participants in adapting to the evolving healthcare landscape, equipping them with the tools to respond to challenges with agility and innovation.

BPCI Advanced will end on December 31, 2025. With only one year remaining in the model, this is a time to reflect and set new goals. As we enter BPCI Advanced’s final chapter, let’s focus on sustaining the momentum and laying the groundwork for future innovations in value-based care.

Key areas of focus:

Sustaining Impact - How can we ensure the transformations achieved through BPCI Advanced continue to thrive beyond the model's conclusion? Participants can focus on leveraging claims data, care-coordination, and patient engagement for future value-based care. By adapting lessons learned and expanding successful approaches, organizations can maintain and enhance their impact.

Exploring New Opportunities in Value-Based Care - How can participants expand on their experience in BPCI Advanced? With the introduction of TEAM (Transforming Episode Accountability Model), participants have an exciting pathway to extend value-based care with CMMI. Beyond TEAM, this is also a chance for organizations to build out initiatives within their organizations, leveraging insights and innovations gained through BPCI Advanced.

Strengthening Collaboration - How can participants further enhance partnerships to drive collective success? In BPCI Advanced we offer learning events to foster collaborative relationships with fellow participants. As we enter the last year of the model, solidifying these relationships may assist in future value-based care opportunities.
