CMS Update to MIPS CQM #497: Preventive Care and Wellness (Composite)
The Centers for Medicare & Medicaid Services (CMS) has received several inquiries regarding the Submission Criteria #6 for Quality Measure #497: Preventive Care and Wellness (Composite) (PDF, 663KB) [ https://qpp.cms.gov/docs/QPP_quality_measure_specifications/CQM-Measures/2024_Measure_497_MIPSCQM.pdf ] (available only as a Merit-based Incentive Payment System (MIPS) clinical quality measure (CQM)).
This composite measure contains 7 component measures that were previously specified as individual quality measures. After assessing the issue,* CMS determined that there are incorrect instructions within the measure specification regarding Submission Criteria #6 (quality measure #226**1** component) that require the reporting of all 3 performance rates*. Rather than submitting all 3 performance rates for Submission Criteria #6 (quality measure #2261 component), only the 2nd performance rate (identified as Submission Criteria #6b within the measure specification) will be required when reporting this component measure within the composite measure for the 2024 performance period. There aren’t any issues regarding the other 6 component measures for this composite measure.
*Reporting the Measure*
All 7 component measures and applicable Submission Criteria are required for reporting this composite measure. To report Submission Criteria #6 (quality measure #2261 component), you’ll *only* submit the 2nd performance rate (Submission Criteria #6b) as documented in the QPP measures [ https://github.com/CMSgov/qpp-measures-data ]repository [ https://github.com/CMSgov/qpp-measures-data ] on GitHub shown below:
* *Submission Criteria #6b “tobacco”: *Percentage of patients screened for tobacco use and, if identified as a tobacco user, received cessation intervention during the encounter or within the previous 6 months.
*Next Steps*
CMS won’t update the measure’s specifications. However, CMS will:
* Provide more information in upcoming Qualified Clinical Data Registry (QCDR) and Quality Registry Support Calls.
* Create an addendum for this measure, which will be added to the 2024 Explore Measures & Activities [ https://qpp.cms.gov/mips/explore-measures?tab=qualityMeasures&py=2024 ] tool and the 2024 MIPS Clinical Quality Measure Specifications and Supporting Documents (ZIP, 56MB) [ https://qpp-cm-prod-content.s3.amazonaws.com/uploads/2630/2024%20MIPS%20CQM%20Specifications%20and%20Supporting%20Documents.zip ].

Second Snapshot of 2024 Qualifying APM Participant (QP) Status and APM Participation Data Now Available in QPP Participation Status Tool
On *October 7*, the Centers for Medicare & Medicaid Services (CMS) updated its Quality Payment Program Participation Status Tool [ https://qpp.cms.gov/participation-lookup ] based on the second snapshot of Alternative Payment Model (APM) data. The second snapshot includes data from Medicare Part B claims with *dates of service between January 1, 2024, and June 30, 2024*.
The tool includes 2024 Qualifying APM Participant (QP) status [ https://qpp.cms.gov/apms/advanced-apms ] and MIPS APM participation status.
To learn more about how CMS determines QP and the APM participation status for each snapshot [ https://qpp.cms.gov/apms/apm-determination-periods ], please visit the QPP website [ https://qpp.cms.gov/ ].
*What Action Do I Need to Take? *
* Log in to the QPP website and verify your Entity information.

To view your QP or APM participation status:
* Visit the QPP Participation Status Tool [ https://qpp.cms.gov/participation-lookup ].
* Enter your 10-digit National Provider Identifier (NPI).
*What Does QP Status Mean?*
If you qualify as a QP, this means you're:
* Eligible for the 1.88% APM [ https://qpp.cms.gov/apms/overview ] incentive bonus;
* Eligible for APM-specific rewards; and
* Exempt from participating in MIPS [ https://qpp.cms.gov/mips/how-eligibility-is-determined ].
