C2C relies on trusted community partners like Dawson County Family Partners, to educate their communities about how to get the most out of their health care coverage with C2C.
Last year, Dawson County Family Partners (DCFP) [ https://www.dawsonfamilypartners.org/ ] in Lexington, NE, distributed C2C materials to residents of their community at mobile food pantries, a YMCA Adult Expo, Back-to-School Nights, and the Dawson County Fair. Resources included the Roadmap to Better Care and My Health Coverage at-a-Glance [ https://www.cms.gov/priorities/health-equity/c2c/navigate-your-coverage ]. Watch this video [ https://www.youtube.com/watch?v=TwTcIvKAdD8 ] to learn how DCFP and other rural communities are benefitting from C2C.
