*                             Collecting ETC Model Participant Information*

If you are or believe you are a Participant in Measurement Year (MY) 7 of the ESRD Treatment Choices (ETC) Model (start date January 1, 2024) please fill out the survey here [ https://app.innovation.cms.gov/ETCModelSurvey ] by 11:59pm ET May 1st, 2025. 

New ETC Participants for MY7 were informed of their participation in the ETC Model by mail in December 2023. If you were an ETC Participant in a previous MY and have onboarded to the 4Innovation (4i) platform previously, there is no need to submit a separate response for MY7. If you believe you are an ETC Participant but are unsure of the MY during which you were a participant, please fill out the survey.

This survey is intended to gather contact information for new ETC Model Participants. CMS will use this information to invite new participants to access the 4i platform for operations, reporting and data pertaining to the ETC model. For MY7, ETC Participants may use 4i to download their performance reports, called the PPA Report, one month prior to the start of the PPA Period 7 on July 1, 2025. ETC Participants must have access to 4i to complete a Data Sharing Agreement which will allow access to beneficiary-level data.

MY7 began on January 1, 2024, and concluded on December 31, 2024. Payment adjustments based on ETC Participant performance during MY7 will be applied during Performance Payment Adjustment (PPA) Period 7, which begins on July 1, 2025. 4i invitations will be distributed to ETC Participants who do not already have an active 4i account in May 2025.

As described in § 512.390(a) in CMS-5527-F, CMS will notify each ETC Participant, in a form and manner determined by CMS, of the ETC Participant's attributed beneficiaries, Modality Performance Score (MPS), and Performance Payment Adjustment (PPA) for a PPA Period no later than one month before the start of the applicable PPA Period.

CMS finalized regulations at §512.390(b) in the CY 2022 ESRD PPS Final Rule stating that the “form and manner” will be via a web-based platform and that ETC Participants will need a Data Sharing Agreement signed to receive beneficiary-level data.
