*In Case You Missed It: **TEAM Participation List and Voluntary Opt-In*

In case you missed it, CMS recently shared exciting news! CMS published the Transforming Episode Accountability Model (TEAM) Participant List, which identifies hospitals by CMS Certification Number (CCN) that are located in mandatory core-based statistical areas (CBSAs). You can easily check the list here [ https://www.cms.gov/team-model-participant-list ] to see if your hospital is included. If you’re affiliated with an acute care hospital, we encourage you to take a moment and verify your hospital’s status. Please note that CMS will publish periodic updates to the list to capture any status changes among hospitals.
*If your hospital is on the list*, we’re requesting that a representative complete the TEAM Primary Point of Contact Identification Form [ https://forms.office.com/g/e3j1wDRrhT ]. It’s a quick and straightforward process to identify two individuals who will serve as the primary contacts for TEAM-related updates and communication with CMS. In the coming months, CMS will be reaching out to provide helpful resources and guidance to support TEAM participants’ preparation for model implementation.
*For hospitals not on the list*, we’ve got more exciting news! CMS announced in the Fiscal Year 2025 Inpatient Prospective Payment System (IPPS) Final Rule [ https://www.federalregister.gov/documents/2024/08/28/2024-17021/medicare-and-medicaid-programs-and-the-childrens-health-insurance-program-hospital-inpatient ] a one-time voluntary opt-in opportunity for hospitals participating until the last day of the last performance period in the BPCI Advanced Model or the last day of the last performance year in the CJR Model, to participate in TEAM. That means to be eligible for voluntary opt-in:
* BPCI Advanced Model hospitals must participate until December 31, 2025.
* CJR Model hospitals must participate until December 31, 2024.
If your hospital is eligible and interested, you’ll need to submit a written participation election letter to CMS between January 1, 2025, and January 31, 2025. CMS will provide all the necessary details about the form and submission process as we get closer to the date.
If your hospital is thinking about opting in, we recommend starting preparations now. The election period opens in January 2025, and opting in will have a significant impact on your hospital’s future participation in these important CMS models. We recommend reviewing the TEAM webpage [ https://www.cms.gov/priorities/innovation/innovation-models/team-model ] for information about the model and checking back intermittently as we provide updated information and resources to help support participation in the model.
