Now Available: Updated 2025 CMS871v4 (Hospital Harm – Severe Hyperg lycemia) Hospital-Inpatient Electronic Clinical Quality Measure Flow

The Centers for Medicare & Medicaid Services (CMS) has updated and re-published the 2025 electronic clinical quality measure (eCQM) flow for CMS871v4 (Hospital Harm – Severe Hyperglycemia) to the eCQI Resource Center [ https://ecqi.healthit.gov/ ]. As background, CMS871 is a ratio measure where calculations are based on the aggregate of numerator and denominator observations, determined after the removal of numerator and denominator exclusions. Additionally, ratio measure calculations yield a ratio, and not a performance rate. Updates to the measure flow correct inaccuracies in the original sample calculation, posted in August 2024.

 The eCQM flows supplement eCQM specifications for the 2025 reporting period for Hospital-Inpatient eCQMs [ https://ecqi.healthit.gov/eh-cah?qt-tabs_eh=ecqm-resources&globalyearfilter=2025&global_measure_group=3716 ].

 The eCQM flows are designed to assist in interpretation of the eCQM logic and calculation methodology for performance rates. The eCQM flows provide an overview of each of the population criteria components and associated data elements. These flows are intended to be used as an additional resource when implementing eCQMs and should not be used in place of the eCQM specifications or for reporting purposes. Specific to the Hospital-Inpatient eCQM flows, a “Read Me First” guide is available to assist users as they navigate the flows.
