Check Final 2025 MIPS Eligibility Status

* Results from the third APM Snapshot will also be available in December 2025 and will be announced through the QPP listserv.

* If you were *initially eligible for MIPS* at a particular practice, your eligibility status could have changed, and you may no longer be eligible at that practice. Check to make sure you’re still eligible. 
* *NOTE: *We’re updating results for the rural special status in the next few weeks based on the most recent Federal Office of Rural Health Policy (FORHP) Eligible ZIP code file available. Please check back at the end of December to confirm whether you have the rural special status for 2025. 
* If you were *initially ineligible for MIPS* at a particular practice, your eligibility status didn’t change at that practice. 
* If you *joined a new practice* (meaning you billed under, or assigned your billing rights to, a new or different Tax Identification Number (TIN)) *between October 1, 202**4**, and September 30, 202**5*, you need to check your eligibility status at this new practice. 
* If you’re a clinician that *joined a new practice after September 30, 202**5*, you aren’t eligible for MIPS as an individual based on your association with that new practice (identified by TIN). However, you will receive a MIPS payment adjustment if the practice chooses to participate in MIPS as a group.

Final 2025 Eligibility and MIPS Value Pathways (MVP) Registrations: Individuals and Groups

Final MIPS eligibility can affect an individual or group’s ability to report the MVP for which they registered.* An individual (identified by TIN/NPI combination) or group (identified by TIN) that becomes ineligible or opt-in eligible when final eligibility is released can’t report the MVP they registered for.* 

Instead, they can:  


* Voluntarily report traditional MIPS. 
* Opt-in to traditional MIPS reporting (if applicable). 
* Do nothing/don’t report. (Ineligible and opt-in eligible clinicians and groups aren’t required to report.) 

Final 2025 Eligibility and MVP Registrations: Subgroups

*Final eligibility **won’t** change a subgroup’s ability to report the MVP for which they registered.* We only use initial eligibility results to determine a subgroup’s eligibility to register for and report an MVP. 


* If the subgroup’s affiliated group becomes ineligible, or opt-in eligible, when final eligibility is released, the subgroup can still report the MVP for which they registered. 
* If the subgroup reports, the MIPS eligible clinicians in the subgroup will receive the subgroup’s score and associated payment adjustment unless they have a higher score under that TIN/NPI combination from individual, group, or APM Entity participation.  
* If the subgroup doesn’t report data, we won’t assign a score to the subgroup and the individually eligible clinicians in the subgroup would need to report another way.

Opt-In and Voluntary Reporting Options for 2025

Clinicians and groups who are identified as “opt-in eligible” on the QPP Participation Status Tool [ https://links-2.govdelivery.com/CL0/https:%2F%2Fqpp.cms.gov%2Fparticipation-lookup/3/0101019388b8296f-b42ffcb1-5a90-4f9d-8674-42c663ba409c-000000/UHPxROvdPqXezsDdAkCulm5BUNYbKVgJ5nzjtZAX4KA=381 ] can: 


* Elect to* Opt-in to MIPS. *You’ll *receive a MIPS payment adjustment* (positive, negative, or neutral). 
* Elect to *Voluntarily Report*. You *won’t receive a MIPS payment adjustment*. 

*Note:* Once you make your election, your election is binding and irreversible. 

Clinicians and groups who are excluded from MIPS and aren’t “opt-in eligible” may choose to voluntarily report data for traditional MIPS (no election required for these clinicians) but they won’t receive a MIPS payment adjustment.

Opt-In Elections

* Qualified Clinical Data Registries (QCDRs) and Qualified Registries can also submit elections on behalf of these clinicians and groups. 
* Elections can be made once the 2025 MIPS submission period opens on January 2, 2026.

Confirm Access to Healthcare Quality Information Systems (HCQIS) Roles and Profile Account (HARP)

In preparation for the 2025 performance year data submission period, which opens on *January 2, 2026*, we encourage you to sign in to the QPP website with your existing HARP account to ensure you don’t lose access. 

If you don’t already have a HARP account and QPP role, please create one. Due to the opening of the 2025 MIPS data submission period, the QPP Service Center is projecting an increase in volume between January 2, 2026, and March 31, 2026, which could result in longer wait times.

Due to the anticipated increase in volume at the QPP Service Center and to minimize a backlog, please use only one method of reporting (email, phone, or CCSQ Support Central) for the same issue. Note: Cases are processed in the order in which they’re received, regardless of how the Service Center was contacted. Please allow time for processing

Submit Your Data Early

We encourage you to submit your 2025 MIPS performance period data early during the submission period. Early submission will allow you plenty of time for Service Center assistance, if needed.
