Now Available: 2025 CMS QRDA I Implementation Guide (IG), Schematron, and Sample Files for Hospital Quality Reporting (HQR)
The Centers for Medicare & Medicaid Services (CMS) has published the 2025 CMS Quality Reporting Document Architecture (QRDA) Category I Implementation Guide (IG), Schematron, and Sample Files [ https://ecqi.healthit.gov/qrda ] for Hospital Quality Reporting (HQR). The 2025 CMS QRDA I IG outlines requirements for eligible hospitals and critical access hospitals (CAHs) to report electronic clinical quality measures (eCQMs) for the calendar year 2025 reporting period for the Hospital Inpatient Quality Reporting (IQR) Program, the Medicare Promoting Interoperability Program, and the Outpatient Quality Reporting (OQR) Program.
The 2025 CMS QRDA I IG [ https://ecqi.healthit.gov/qrda ] contains these high-level changes compared with the 2024 CMS QRDA I IG:
* Updated eCQM information to align with the updated version for the 2025 reporting period.
* Removed the “Submitting the encounter id associated with a CCDE” section.
* Updated the validation rule CMS_0063 to be applicable for inpatient eCQMs submissions only.
* Updated the validation rule CMS_0084 on the Medicare Beneficiary Identification Number (MBI) requirement for hybrid measures submission.
There were no changes to the 2025 CMS QRDA I Schematron File [ https://ecqi.healthit.gov/qrda ] compared with the 2024 CMS QRDA I Schematron File
The 2025 CMS QRDA I Sample Files [ https://ecqi.healthit.gov/qrda ] updates include:
* Updated eCQM information to align with the updated versions for the 2025 reporting period.
* Removed the use of "relatedTo" template from the hybrid measure sample file.
* Added new sample files to demonstrate reporting CMS1074v2 and CMS1026v2.
