CMS is announcing the release of the final determinations for the HCPCS Level II Q1 2025 coding cycle.

The publication covers a range of crucial updates, including new codes, revisions to existing codes etc. For each application, the following information is provided:


* A statement of the topic/issue
* A summary of the applicant’s request
* CMS’ coding determination
* Please note this publication involves significant changes to existing HCPCS Level II codes that describe various Vancomycin Hydrochloride products. Please refer to Appendix A for details regarding these changes.
