The Quality Payment Program (QPP) Small Practices Newsletter is a monthly resource that provides small practices (15 or fewer clinicians) with program updates, upcoming QPP milestones, and resources to support their continued participation and success in QPP. Data Submission Window for 2024 Performance Year Closes April 14, 2025

The data submission window for the 2024 performance year has been extended. You can submit data for the 2024 performance year until the submission window closes on *April 14, 2025, at* *8 p.m. ET*.

*How to Submit and Review Your 2024 Merit-based Incentive Payment System (MIPS) Data*

Follow the steps outlined below to submit data:


* Go to the QPP sign-in page [ https://qpp.cms.gov/login ].
* Sign in using your QPP access credentials.
* Submit your data for the 2024 performance year or review the data reported on your behalf by a third party. *(You can’t correct errors with your data after the submission period, so it’s important to ensure the data submitted on your behalf is accurate.)*

*How to Sign in to the QPP Data Submission System*

To sign in and submit data, clinicians will need a Health Care Quality Information Systems (HCQIS) Access Roles and Profile (HARP) account and a QPP role. For help enrolling with HARP, please refer to “Step 1. Register for a HARP Account” in the QPP Access User Guide (ZIP, 4MB) [ https://qpp-cm-prod-content.s3.amazonaws.com/uploads/2955/QPP-Access-User-Guide.zip ]. For help obtaining a QPP role, please refer to “Step 2a. Connect to an Organization” in the QPP Access User Guide (ZIP, 4MB) [ https://qpp-cm-prod-content.s3.amazonaws.com/uploads/2955/QPP-Access-User-Guide.zip ].

The Centers for Medicare & Medicaid Services (CMS) encourages all users with an existing HARP account to sign in to the QPP website [ https://qpp.cms.gov/login ] now to ensure they don’t lose access.

*Note: *Clinicians who are unsure about their eligibility to participate in MIPS for the 2024 performance year can check their final eligibility status using the QPP Participation Status Tool [ https://qpp.cms.gov/participation-lookup ]. Clinicians and groups that are opt-in eligible will need to make an election to opt in or voluntarily report before they can submit data. (No election is required for opt-in eligible clinicians and groups that don’t want to participate in MIPS.) *Reminder: Preliminary Scoring No Longer Available During Submission*

As a reminder, CMS eliminated the preliminary score and preliminary category-level scores from submission. We’ll release 2024 MIPS final scores in mid-June 2025.

*Submit Your Data Early*

We encourage you to submit your 2024 MIPS performance period data early during the submission period. Early submission will allow you plenty of time for QPP Service Center assistance if needed.

*Upcoming QPP Service Center Hold Times*

The QPP Service Center is projecting an increase in volume between now and the close of the data submission window for the 2024 performance year on April 14, 2025, which could result in longer wait times.

CMS recommends the following to minimize wait times and ensure successful 2024 data submission:

*Use One Method to Report Issues *

Due to the anticipated increase in volume at the QPP Service Center and to minimize a backlog, please use only *_one_* method of reporting for the same issue (email, phone, or CCSQ Support Central).

Note: Cases are processed in the order in which they’re received, regardless of how the QPP Service Center was contacted. Please allow time for processing.

2024 MIPS Extreme and Uncontrollable Circumstance (EUC) Application Extension

CMS reopened the 2024 MIPS EUC Exception Application window in response to the national IV fluid shortage; therefore, *_CMS will only accept applications citing the shortage as the basis for requesting reweighting under the MIPS EUC Exception_*. Any applications submitted for reasons outside of the national IV fluid shortage will be denied.

Individual MIPS eligible clinicians, groups, virtual groups, and APM Entities participating in MIPS (including Shared Savings Program Accountable Care Organizations) affected by the national IV fluid shortage will be able to submit a 2024 MIPS EUC Exception Application *until April 14, 2025, at 8 p.m. ET.* They can apply to request reweighting for any or all MIPS performance categories. 

For example, you may need to apply if your practice’s administrative and support staff has needed to focus on supply chains to support patients’ care, affecting their capacity to collect and prepare data for reporting.

2025 MIPS Value Pathways (MVPs) Registration Now Open

The MVPs registration window is open for the 2025 performance year. Individuals, groups, subgroups, and Alternative Payment Model (APM) Entities that want to report an MVP can register *until December 1, 2025, at 8 p.m. ET*.

*Note:* If the Consumer Assessment of Healthcare Providers and Systems (CAHPS) for MIPS Survey is an available measure in your selected MVP and you want to administer it as 1 of your 4 required measures, you‘ll need to complete both your MVP registration and a separate CAHPS for MIPS Survey registration by *June 30, 2025 at 8 p.m. ET*. 

To register, you’ll sign in to the QPP website [ https://qpp.cms.gov/ ]with your Health Care Quality Information Systems (HCQIS) Access Roles and Profile (HARP) [ https://qtso.cms.gov/access-forms/harp-account-registration ] account.

You must have a HARP account and QPP Security Official role to complete the MVPs registration.

2025 Consumer Assessment of Healthcare Providers Survey (CAHPS) for MIPS Survey Registration Now Open

Registration is open through *June 30, 2025, at 8 p.m. ET* for the CAHPS for MIPS Survey [ https://www.cms.gov/data-research/research/consumer-assessment-healthcare-providers-systems/cahps-mips ] for the 2025 performance year.

Groups, virtual groups, subgroups, and APM Entities with 2 or more clinicians (including at least one MIPS eligible clinician) can register to administer the CAHPS for MIPS Survey [ https://www.cms.gov/data-research/research/consumer-assessment-healthcare-providers-systems/cahps-mips ] for traditional MIPS [ https://qpp.cms.gov/mips/quality-requirements?py=2025 ], APM Performance Pathway (APP) [ https://qpp.cms.gov/mips/app-quality-requirements?py=2025 ], or MVPs [ https://qpp.cms.gov/mips/mips-value-pathways ] reporting. 

*Who doesn’t need to register?* 


* Medicare Shared Savings Program Accountable Care Organizations (ACOs) [ https://www.cms.gov/medicare/payment/fee-for-service-providers/shared-savings-program-ssp-acos/about ]
* These ACOs are required to report through the APP so they’re automatically registered for the CAHPS for MIPS Survey, which is required under the APP.
* These ACOs don’t need to register but will still need to hire a CMS-approved vendor to administer the CAHPS for MIPS Survey.

*Who must register?* 


* Groups and APM Entities (other than Shared Savings Program ACOs) that choose to report through the APP must register because the *CAHPS for MIPS Survey is a required measure under the APP*.

*Who can register?* 


* Groups, virtual groups, and APM Entities that intend to administer the CAHPS for MIPS Survey as 1 of their 6 quality measures for reporting traditional MIPS
* Groups, subgroups, and APM Entities that are registered to report the following MVPs [ https://qpp.cms.gov/mips/explore-mips-value-pathways?py=2024 ] and that intend to administer the CAHPS for MIPS Survey as 1 of their 4 quality measures:

* Advancing Cancer Care MVP [ https://qpp.cms.gov/mips/explore-mips-value-pathways/2024/M0001 ]

2025 MIPS Quality Benchmarks Available

The 2025 historical benchmarks for quality measures [ https://qpp.cms.gov/benchmarks#benchmarks-2025 ] are now available on the Benchmarks webpage [ https://qpp.cms.gov/benchmarks ] of the QPP website. Benchmarks are the point of comparison we use to score the measures you submit. Your performance on each measure is compared to its benchmark to determine how many points the measure can earn.

The goal of this survey is to help the Centers for Medicare & Medicaid Services (CMS) better understand the experiences of QPP participants who have submitted Performance Year (PY) 2024 data. Your responses will help CMS identify how the submissions process can be improved, as well as what’s working well.

Results will be reported in such a way that no single individual, group, or entity can be identified. This survey is conducted by outside contractors, and participation is completely voluntary and confidential. Your decision to participate or not to participate in this survey will not affect any pending or future determinations. This survey should take no more than 15 minutes to complete.

Your feedback is essential to understand how we can best serve QPP participants. Eligible clinicians who complete this survey may receive an Improvement Activities credit. If you have any questions, please reach out to qppuserresearch@cms.hhs.gov.
