_*FY 2025 ICD-10-PCS Procedure Code Files*_
We have provided replacement Index Addenda files in the Addendum link and a replacement Table and xml file in the Code Tables and Index link on our CMS web site in the Downloads section at: https://www.cms.gov/medicare/coding-billing/icd-10-codes/2025-icd-10-pcs to address a typographical error and to correct the function of the hyperlinks on the Introduction page of the Tables and Index file.
These FY 2025 ICD-10-PCS codes are to be used for discharges occurring from October 1, 2024 through September 30, 2025.
_*FY 2025 ICD-10-CM Diagnosis Code Files*_
The FY 2025 Code Tables, Tabular and Index files, which contain information on the ICD-10-CM updates for FY 2025, are available in the Downloads section at: https://www.cms.gov/medicare/coding-billing/icd-10-codes/2025-icd-10-cm. These FY 2025 ICD-10-CM codes are to be used for discharges occurring from October 1, 2024 through September 30, 2025 and for patient encounters occurring from October 1, 2024 through September 30, 2025.
The FY 2025 ICD-10-CM Coding Guidelines, FY 2025 Conversion Table and FY 2025 Present on Admission (POA) Exempt Code List are also available.
All diagnosis code files will also be made available on the CDC/NCHS website at: https://www.cdc.gov/nchs/icd/icd-10-cm/files.html.
_*Upcoming ICD-10 C&M Committee Meeting*_
*Friday, December 6, 2024* is the deadline for members of the public to submit code requests for discussion at the March 18-19, 2025 ICD-10 Coordination and Maintenance Committee Meeting. Please indicate if you are submitting your request for consideration of an October 1, 2025 or an April 1, 2026 implementation date.
* CMS will only accept *ICD-10-PCS procedure code request *applications submitted via the Medicare Electronic Application Request Information System™ (MEARIS™). The ICD-10-PCS code request application can be accessed at: https://mearis.cms.gov [ https://mearis.cms.gov/ ]. Requests submitted through the ICDProcedureCodeRequest mailbox will no longer be considered. Within MEARIS™ we have built in several resources to support requestors:
* Please refer to the “Resources” section for guidance regarding the request application submission process at: https://mearis.cms.gov/public/resources.
* Technical support is available under “Useful Links” at the bottom of the MEARIS™ site.
* Request application related questions can be submitted to CMS using the form available under “Contact” at: https://mearis.cms.gov/public/resources?app=icd-10-pcs.
* The time required for request submission, including the time needed to gather relevant information as well as to complete the form may be extensive depending on the nature of the code request. Requestors are therefore encouraged to start in advance of the due date to ensure adequate time for submission.
*Diagnosis code *requests should be directed to NCHS at nchsicd10cm@cdc.gov. For additional information, visit https://www.cdc.gov/nchs/icd/icd-10-maintenance/proposals.html.
